Position Description: Case
Manager
The
Case Manager will provide client
advocacy, community resource education and linkage, and drug-testing monitoring
while working within a multidisciplinary team to contribute to appropriate
client care and treatment guidance.
Essential Duties
This position includes, but is not limited to, the
following essential functions:
- Manage and coordinate
case load and maintain all client documentation in the electronic record within
established timeframe.
- Attend staff/supervision/agency
meetings and trainings within established time frame.
- Comply with all HIPAA regulations in coordination with the HIPAA
Compliance Officer.
- Abide by all Trivium Life Services Corporate
Compliance policies and Code of Ethics.
- Monitor client attendance and treatment progress as well as
communicating with the treatment team and individual therapist about client's
progress. Working with all staff and within multidisciplinary teams to
contribute to appropriate wrap-around services specific to each client.
- Coordinate resources and referrals with community partners to
assist client in obtaining basic needs and other case management goals.
- Monitor client drug testing as applicable.
- Complete Service Plans and case management assessments in accordance to cmpany policies and procedures within established timeframe.
- Complete monthly PO reports.
- Contribute to developing an ongoing resource database and
networking with community partners to build relationships while representing
Trivium Life Services.
- Other duties as assigned.

These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the Case Manager's job. While performing the
responsibilities of the Case Manager job,
the employee is required to talk and hear.
The employee is often required to sit and use their hands and fingers,
to handle or feel and to manipulate keys on a keyboard. The employee is often required to stand,
walk, reach with arms and hands, and may kneel or crouch. Vision abilities required by this job include
close vision.
Working Conditions
Working conditions are normal
for an office environment. The noise
level in the work environment is usually quiet to moderate. Work may require occasional weekend and/or
evening work. Per state territory
requirements, must be able to remain on-call and available to provide staff
support in event of emergencies. In
addition, must be able to travel between locations and occasionally attend
seminars which may require overnight stay.
Requirements/Qualifications
- Bachelor's degree
from an accredited college or university in a human services field.
- Experience in
behavioral health preferred.
- Valid Driver's or Chauffer's
license (per state territory requirements) and a driving record that is
acceptable to Trivium Life Services.
